Do you need supportive home care for an adult with disabilities?

Aptiv’s Supportive home care program assists adults with disabilities through tasks related to their well-being and day to day life. This is a “do for” service, meaning that the staff performs tasks an individual is unable to do without assistance. This service does not require a development plan.

Supportive Home Care service options

Supportive Home Care options may include but are not limited to:

  • Eating

  • Dressing

  • Exercising

  • Transfer/Ambulation - ex. Movement from bed to wheelchair

  • Medication reminders

  • Getting ready for the day

  • Meal preparation

  • Supervision/Monitoring

  • Accompanying individuals to community activities

  2. Aptiv meets with the individual and their support network to develop an individualized plan and provide services as outlined in that plan.

*Most individuals participating in Aptiv's Supportive Home Care services receive funding through Family Care or IRIS long-term care services. Private Pay option is available.

a Medicaid managed long-term care program for frail elders and adults with disabilities. The program provides a wide range of health and long-term care services.
